Billing Information

1. Oregon University System customer: we need a valid "index" to set up an account.
2. Government agency (non-Oregon University System) customer: we need hard copy of the PO prior to set up active account or payment in advance.
3. All other US customers: either hard copy of the PO with credit application or payment in advance.
4. Outside the US: payment in advance only.
